Your Medical Records

Medical records are confidential. We will not release information about your care to anyone, including other family members, without a signed release. For copies of medical records to the individual patient, the processing fee is $10.00 and $.50 a page for the first 50 pages and $.25 per page thereafter. Virginia Code 8.01-413a. Please allow 2 weeks to process these requests.

Copies of medical records to attorneys, insurance companies, or other third parties will be subject to reasonable fees.

There is a $20.00 fee for each medical form that needs to be filled out.
